Cleared Accenture Skill interview by answering almost all the questions and received congratulatory mail for HR round and submitting all documents. Then after a week received a call from HR saying that due to some technical issue, the skills round will happen again and an online assessment was set up. In the assessment, questions which were not directly related to the profile were asked and then a rejection mail was received. Does this happen a lot?

...a recruiter I actually know reaches out to me cold, out of blue, asks if I’d be interested in initially exploring...yes...this very same job I’d spent the last 6 months “exploring” and allegedly “winning”. Meaning, they’d engaged a new recruiter and were basically starting over while trying to keep me on the hook as the “top/leading” candidate by saying a decision would be very soon. Maybe tell recruiters who you’ve spoken to/interviewed first?? I don’t think i was entitled to this job or anything, but this felt kinda amateur hour if you ask me.
Recruiters tell dozens and dozens of people they are in the top 5. It makes you feel like you want to keep working with this person because their “inside info” = access. It’s all baloney. Never trust anything a recruiter says until you actually get an offer.
