Get a new job, set a meeting with your boss to tell them when you’re starting your new job and put that info in writing to HR. Don’t quit. Share the good news about your new job.

2 weeks is standard. Resign to your direct manager first then follow up with written email. Ask your direct manager how he/she wants to manage the news to internal/clients. Whatever the reason you’re leaving, keep it professional and don’t burn bridges.

But in terms of these specific questions, you’d first schedule a meeting with your boss. The first sentence is probably the hardest to get out in terms of conversation, but the rest of the conversation typically naturally flows. They’ll ask when you envision your last day being - two weeks is the unwritten rule but if you love your team/this was a hard decision/it’s during a particularly insane time of year, any extra time above 2 weeks will be seen as you being generous/will earn you points in the event you may want to return or work with this people later. How people leave in the end sticks with people (including how they behave during their last two weeks).
Then once you leave the meeting with boss, you immediately email HR disclosing that you have resigned, boss has already been informed, and xyz is your last day. Boss will probably also email HR, but doesn’t hurt for your records and is probably more top on your list than boss’
2 weeks is standard. Don’t forget that they would NOT do the same for you.
Where I’m from, two weeks is the unwritten rule. If you want to leave a good impression you can give three weeks. Tell your boss and write out a letter stating your last day and sign it. Then send to HR as well. Not sure on what you can say for your reasoning, how long have you been there? If it’s short then you can say you don’t feel as though you are a good fit. Or if you’ve been there a while you can say that you just want a new opportunity for growth.
You absolutely don’t but if you want to those are sometimes decent ones I’ve seen used that stay fairly neutral.
You do not need to give a reason. Period. Also 3 weeks is generous and demonstrates your commitment to the business. On a toxic team or bad relationship that’s very professionally mature of you.
Have your email to Hr drafted and send it just after telling your boss. The last time I resigned, I told my president then send him a follow up email immediately after we met and I copied my HR rep.
Take your PTO owed. Back up all your digital files. Take care of your health care with appropriate sick leave. No need to take unfair advantage.
Connect with great coworkers on LinkedIn and ask/give recos.
Once all that is done. Send a professional letter to manager and HR. Give 2 weeks, more if there is a need. No need to give reason. If asked respond with, “I got a new opportunity.”
Do not burn bridges. Invite everyone to goodbye drinks or lunch. Good people will show. The people whom you dislike will generally not.
2 weeks only. Never offer 3.
