I don’t disclose a “reason” for PTO. You don’t need a reason. I put the dates I’ll be out on the team calendar, and I enter the dates in WorkDay for my manager to click “approve,” and that’s it. We don’t even talk about it.
Your manager shouldn’t care what you use your vacation for. The only reason to have an approval method is so that a manager can coordinate everyone’s time off and make sure there’s sufficient team coverage. It’s for logistics reasons. It’s not to approve your reason for taking it.
No details at all. There might be informal chat after like “have anything nice planned?” But not linked to accepting or rejecting pto request
No detail at all, I state the dates I need off. If they ask later I’ll share but not in the actual request.
It depends. If I would have the ability to jump on in the event of an issue, I’ll let my direct team know but not anyone else. Also, if I’m visiting family/something simple.

Depends how close I am with my boss…
I keep it simple in the actual request but I’m friends with my boss so usually we talk about our vacation plans throughout the year
