I went through this, it’s awful. I finally pulled the person aside and said you gotta cut it out, you’re freaking people out. She got the message and quit talking about it in such graphic detail. If that tactic doesn’t work then I’d let the manager handle it.
This would be my suggestion as well. If you're comfortable with it, it may be "easier" for them to get it if they hear it from a peer. That being said it's also totally understandable if you'd rather go to a supervisor or your HR department.

Been there both professionally and personally. In both instances I had to say something and in both cases they just didn’t get it. Sadly in both cases they took it as a challenge to do it more. For work, I went to their supervisor. I hate to be someone to do that, but it had to be done. They were mad, but at least I got relief from the stories, and sometimes pictures.
Just tell the person it grosses you out, and you don't want to hear it. I'm sure you'll be more tactful, but those are the basics of it. It sounds like this person does care what you think. Otherwise, they might not tell you such personal things, so it should be fine. It's a worse scenario in your head than it will be in real life.
