We don't have 90% of the information. Do you like the guy? Do you think they are qualified for the role? Have you stayed in touch? Was he good at his job when he worked there? Why did they leave previously? Do you want to work with them? Would you want them on your team? If the answer to the above is no or bad, then I wouldn't feel an obligation. Also staff versus manager has quite a different weight behind it.
OP, shouldn't be awkward for you. He's asking to come in as an M. If he reports to you, just make sure to respect the knowledge he brings in. Help him get promoted faster. I've been older than my Ms for years. My best Ms are the ones that have me act as the M.
So he helped you out and is now asking for help in return? Seems pretty simple to me.

Eh, maybe weird for a couple days but if he's going for a manager role, he is signing up for it. No different than if one person get promoted and another doesn't. Got to just get over it. Clients create enough problems already.

It's confusing that op is a high ranking adult who feels awkward being in a position of power over someone they haven't worked with in over a decade. Usually people who are that successful wouldn't be weirded out by this. It's just unusual.

I knew senior managers here who were partners elsewhere, earlier in their career - and there were partners here that had worked for them.
At senior manager and above, this isn't uncommon
