Make it a regular habit of checking in with your team lead and manager. Ask these questions:
1. How's my performance?
2. What are the improvement areas?
3. Am I on track for getting the best rating?
Set the expectations clear and work in that direction. Communication is key.
Sadly, in India, the general tendency is to make a meal out of little misses(that can otherwise be swept under the carpet) just to lower ratings/hikes. So, don't be disheartened. Just be more vocal about getting the best rating. You've got to be DHAAKAD!
Pro advice!
Analyze carefully to what your manager/lead is saying is it true. If yes then try to improve, also document your day to day work(activities done in a day) so that later you can refer to the same and communicate to manager/lead for all the stuff you have done.
If there is something to be improved from your side, do take necessary actions by going through code or enhancing your technical skills, else you will face this problem everywhere.
Additionally you should start preparing for the interviews and try to change the company.

Leave, there's plenty of fish in the sea, see the simple concept is "Your manager, cares about himself(his growth, his ambitions etc), and he'll act in accordance with that, always, without a doubt; so you have to take care of yourself". Simple.
