I interviewed with Firm A months ago and accepted a position to start this upcoming fall. Group leader called me to offer the position back in November 2020. Besides interviewing with partners at the firms, and diligent follow ups notifying them of my interest to get started, no communication has transpired since.It turns out that I accepted a better offer w/ Firm B for personal reasons and won’t be going to Firm A. What is the best way to notify Firm A? Call/email group leader? Call/email HR?

I had this happen to a good friend - additional 3rd round because the 2 people in her 2nd round did not agree on outcome (one really wanted her in, other did not). Maybe try to figure out which interview went best and copy that (and try understanding what might have gone wrong in the other one)?
In any case prepare just like the other rounds, our interviews all have the same standard format

I do believe that is common when the 2 interviewers do not agree on the outcome.
I would guess it'll focus a bit more on the points they didn't agree on (e.g. Maths or smth). Good (prob. biased) news is that everyone I've met that faced this situation got the offer (N=5).

Tried asking for feedback from recruiter but only got very general stuff so not sure
It's good news. They want to hire you which is why they are giving you one more chance. You cannot have the same issues across all the interviews so they want you to do well on whatever that one pattern was.
Yeah I had asked recruiter and she was very explicit about not wanting to give specific feedback and just gave me some general advice
I had a friend interviewing for Bain who was given this same situation - it is likely to just be an additional behavioral check (but obviously check feedback to see if their concern was on cases or PEI).
