It is not very easy if you are staying at same firm but very easily doable if you jump firms. One of my friend who was not even thinking about becoming manager just took an offer from a competitor to become manager in 4 years. I have seen people making manager in less than 3 years because they jumped firms twice.
As for your question for salary, if you stay same firm the a first year manager (after 4 years) is most likely going to make less than a first year manager who made manager after 5 or 6. If you jump ship, then you can even make more than than those who made manager after 5 or 6 depending on how good you are at selling yourself.
Why do you want to put the extra pressure on yourself?
The pay usually is the same. If you are market based each level has a pay band and your performance for a year determines if you are in the top, middle, or bottom. So if you were at the top of your experienced senior pay band you may not see as big a salary bump to manager as another senior who was average.
i found that youll move up fast if you are a favorite of the senior leadership. it also depends on the practice
not sure, but i assume it all depends on your performance and previous salary. i dont think that have a standard amount (i.e. not all first yr managers have a 100k salary, more likely a range from xxx to xxx)
