She is insecure and she is letting it show. More than insecure i would say paranoid.

Maybe have a conversation with her and see what she says. She may not know that it’s weird it could just be her personality.
If that is your only work problem then things sound pretty good!
go to the higher up above her or file a complaint with HR asap. That will snap her into shape
That’s so true. Never trust HR. They are only there to protect the company. Not advocate for the employee as they pretend to be.
Time to leave.

I had an inexperienced director and I’m extremely blunt where he’s coming up short, but I articulate it so he understands where he’s falling short. After a year of giving him feedback, he’s doing the same to me. I’ve trained him well and his feedback is on point. It’s important how you explain it so it’s not condescending, rude, lacking in respect. For example. I see you didn’t shave today, do you still enjoy working here. My hygiene isn’t a reflection of my job satisfaction. If you’d like some assistance in conducting an employee sentiment survey I can help you create the survey. I’m surrounded by smart people with weird vibes.

Could you perhaps ask why she feels the need to ask after you like this, when respectfully you have answered/explained? It might help to get both of you on the same page based on each other's perspective, and clear up any misconceptions that neither of you might even realize exist right now. It could be something, it could be nothing. This suggestion may be a reach but if something is irritating you enough to make you feel negatively towards her and/or you working there, then I'd want my staff to address it with me for sure. No-one likes negative or uncomfortable feedback, but I would rather get it before things escalate or cause any undue tension. I do understand that not everyone would be comfortable doing this, nor would some people think it's necessary, but I guess it depends on the person whom it is impacting to make that call.
