I would probably just ask directly about onboarding since I think that would show initiative on your part, not weakness. I just wouldn't bad mouth the previous job since that might come across as you not taking accountability.
I will say, it’s really unfair that we have to protect companies for their bad behavior and take all the accountability when things don’t work out. I agree with your statement because that is the way it is, but it isn’t right.

I would ask about onboarding by framing it as wanting to succeed quickly. I usually say I do my best work when expectations, training, and early support are clear. That shows ownership, not hand holding.
As a hiring manager, I actually like when candidates ask about onboarding. You can frame it as wanting to ramp fast, not needing babysitting. Something like, “What does success look like in the first 30, 60, 90 days?” tells me you’re proactive and serious.
