It happens all the time and yes, every workplace explains it differently. I wouldn't be encouraged by it, per se, but I wouldn't bail on the opportunity over that alone. There'd have to be bigger, more significant red flags for me to withdraw myself from the process.
I worked at an amazing place, and there was a period of time where we had arevolving door of different managers. Personal reasons, another offer, moving, nothing was a red flag, but it’s hard to say what happened here. I’d be mindful and watch for anything else that seems odd.
People leave roles for various reasons. Your initial instinct of being hesitant is warranted and it does not hurt to ask why they left or are no longer there. If it was performance issue you can turn a negative into a positive during discussions and ask about other employees or turn over in the company. This will show them your desire to want to stay long term which is a positive. Remember they want you to feel confident about your decision as well of you decide to take the position.
