It's very difficult. I feel the same and when I'm exhausted, I just ask for 3 days off. It's not enough but I'm trying that way until I get back to my freelance job.
Generally speaking (depending on where you live), taking a few days off every 3 months is not super unusual.
Other than that, two things:
- "A break" is a very vague term. Find out how long you need to take off and how to best use that time off to recharge, because often the way people take their holidays does not adequately serve them to recover. Even for allistic people the general consensus is that a holiday should be at least 2 weeks at a time (and ideally longer than that) to be effective.
- if you NEED a break from work every three months, that may indicate that your work time is not structured sustainably for your needs. So find out what it is that exhausts you and how you can restructure your work-life so you don't run your mental and physical batteries down (e.g. fewer hours, same hours but fewer days, different work environment, home office/hybrid). Also check how you spend your time off work - are you actually allowing yourself to recharge?
For both points, advocating for yourself and honestly explaining your particular circumstances to your boss can help improve your chances - it's an accommodation, not a favour. Depending on where you are based there can even be services in place to help you with the restructuring.
