If you’re struggling and you don’t take a leave to improve your health, is your performance going to be bad enough to get fired? Is your health going to get worse? Seems to me there’s also possible consequences of working while unwell.
4 weeks away is doable. Suck it up and then take time, unless of course you really can’t. What about a short vacation?
Can you take vacation leave in between to help out? Not sure how things work in US but that would really suck if people under 1 year get zero protection...
FMLA is job protection, but it’s still usually frowned upon to fire someone for taking sick time or PTO while under a doctors care. Most companies want to avoid wrongful termination lawsuits despite at-will employment laws. I would suggest getting a doctors order for leave.

Talk to your engagement leader about taking a week of PTO and go from there. Or maybe even working a reduced schedule if that’s possible..
Definitely use PTO to bridge some of the 4.5 weeks until you have a year. 4 weeks may feel like an eternity if you are sick, but you are SO CLOSE! Definitely talk to someone at work also about options. Most people are compassionate and understanding and will try to help you sort something out.
I’m not sure what your illness is, but filing for reasonable accommodation with ADA may be an option for you. I’d suggest reaching out to your people consultant to talk through.
Are you in the US? If so, how close are you to your 1 year mark?
My recommendation - stretch your vacation, sick days, holiday to get you to your 1 year mark. From there you can formally request FMLA with HR with full protections
