Is it a very large company? It’s possible that budget restrictions require the job rec to be passed off to another group with more immediate need so delay to hire will occur or a nepo candidate got ninja’ed into the situation. It all depends on company and culture and budgets.
I am sorry about that. Nothing is a sure thing. Still worth it to take a minute and follow up, just don’t wait around on anything ever.
Pump the brakes. There are any number of reasons they could have temporarily gone silent. Maybe they had something else come up that demanded their attention. Maybe they needed to confer with someone else before making you an offer and haven't been able to yet. Maybe it just slipped their mind. No matter what, a week really isn't that long to go without hearing from them, even in this situation where a call was promised. Send a follow up and see what happens.
