I am confused which should I select, trusting Cognizant and joining on less or going for one the company mentioned above.

Received my first prenatal massage at 19 weeks today and the therapist was trained and did a bit of myofascial stretching techniques. Never touched my stomach. Just lower back, arms, legs and feet. During h1b extension , will I get another 4.5 years of extension? How does extension work when u switch visas?
