There’s a lot of them but lack of humility and trust are pretty high up the list.
For me it's gossipy behavior and/or seeing a supervisor acting two-faced to someone else, and then realizing they probably do the same to me. When I can't trust someoen to tell me what they mean (and I see that they will go complain about it to someone else), I really lose all sense of trust and start to live in fear of my interactions with that person.
Passive aggressiveness and favoritism.
Micromanaging or a complete lack of empathy
I agree with you that muddled thinking is a toxic trait. But for me, the most toxic trait would be dishonesty. One you deal with someone who isn't honest, and you know it, it's impossible to respect them. And workers pick up on that.
