For provider it's a lot different. You can work at a large hospital system and do internal consulting, process improvement activities, project management etc. but I am not sure the pay or opportunities will be on par with consulting or payer. Yes there are huge hospital networks but without an MD or medical type degree the ladder can be a little limiting (again personal opinion) since everyone you would work with would want that from you. Not all providers are the same (ex Kaiser) which would likely have more opportunities for consultants on the provider side since it works with the payer/business side as well and because it is so massive. Pharma is a different world and caters well to post consulting opportunities but also recruits top talent so it can be very competitive.
So since no one else is going to take a stab but a lot seem interested I'll try and answer to the best of my abilities. For payer, it can be a lot of things. Typically you should try and find a client or group you like and find someone you want to work for. Culture is big here so try and find a good fit. Obviously your level in consulting matters. Try to jump ship at least at manager level, anything lower at a payer going into industry is going to be rough (aka you want direct reports so you don't do bs work). With all the payers converging right now there can be a lot of opportunities but also just very few exit opportunities to select from since they are all merging. I imagine the goal would be a bus dev or internal strategy role but this will require lots of networking and having very relevant consulting experience.

@EY2 excellent! I only add that HC going through the bubble of baby boomers (perishable product). So, plan your future move accordingly. If you have a science or engineering degree, consider biotech and medical devices as well.

I know some top Pharma companies have internal consultants. They don't travel but rather stay in an advisor-like role where the leadership is located
