Working in a rural area definitely comes with its own set of challenges—limited resources, long travel times, and high expectations from patients who often can’t access specialized care. For example, we don’t always have certain medications or equipment on hand, and it can take days to get what we need. I often feel like I’m wearing multiple hats, and the pressure to do more with less is real. How do you all manage the high demands while staying sane? Any tips for handling resource shortages or patient expectations when options are limited? Would love to hear how others cope!

Desi folks - question on travelling outside US while you're on L1B visa. I have my valid visa stamp till 2022. Do any of you know if we can travel to India in Dec and come back in Jan? Or is it affected by the Trump executive order that was passed in Jun that said non-immigrant workers can't enter the country till Dec 31? How would you handle not getting a lunch break when you are a salaried employee treating patients for 40 hours a week? I don’t get documentation time either, but all notes are to be completed the same day. Some of us have reached out to HR and we’re told to discuss it with the regional director, but the regional director says we’d have to come in early or leave late to block out a lunch break. My commute is an hour each way, so I’d prefer not to make my day any longer. Advice?

After months of depression and suicidal thoughts, I had a big realization that pulled me out of it.. that I’m alone in the world and I can’t rely on anyone else but myself. As a chronic people pleaser, this was liberating.

I’m having a really hard time with my changing body. 13 weeks, I don’t have too many bad side effects of pregnancy or anything so I consider myself lucky (no nausea ever etc) but I’m depressed at work, my skin is completely broken out and showing new broken face capillaries, and I’m not showing a bump yet so I just feel fat and big. I don’t know where this “glow” is but I look in the mirror and literally am grappling with not recognizing myself :( it’s been impossible to find any clothes too.

I'm a PA in the PNW working for a private group staffing ED's in several hospitals. Recently the group has asked the PA's to take on a couple new roles (triaging and rapid track from triage i.e. straightforward ESI 4, 5). With these changes the PA's are considering asking for increased pay in the form of an addition of a COLA (cost of living adjustment) to our normal salary. Was wondering if any other PA's in the ED have these in their contracts and at what rate.
