Depends on firm. At cap 1 is the best 5 is the worst. Default for first years is 3 unless you did something extraordinary.

Gross how everyone jumps immediately to her doing something inappropriate. Sure, that could be it, but it could also very well be something else that’s either firm’s fault or out of her control. Like she happens to be on a critical project at the moment so they’ll say anything to keep her for the next 3 months and cut her loose after. Or she had already given notice so they didn’t give her a lay-off package, but then realized they did too many cuts and now are trying to keep more people on board. Maybe she’s leaving for a better competitor and that hurt someone’s ego. Maybe her low score was partly due to retaliation, discrimination or something else from a previous manager and this is an effort to resolve it. Or yes, maybe she’s the granddaughter of your biggest client and had sex with all the managing directors in your office. Tons of possibilities, why bother getting yourself worked up over it? If you feel like you need more context to serve as her career advisor you should quietly ask one of the people trying to get her to stay. They may not tell you the truth but whatever they tell you should give you some clues.
