What factors have gone into determine they won't "work out"? What training and coaching and guidance has been made available to them? What biases have they been subjected to that non-POC team members haven't been?

3 out of 4 are bad hires? Without all the details my default opinion is the problem is more related to the manager.
As stated above, figure out why they are not working out and what support you could offer to combat this. Surely, out of three, at least one should work well with the right support. If not, ask yourself how your team managed to make three poor hiring decisions in a row.
Great point! If a team has 3 of 4 doing something wrong that says more about the team than those 3 people.

IF we are to assume you all did the right assessment matching their skills to the role then it sounds like you all failed to explain the vision of the company and also failed to provide the proper motivation for them to work towards the team’s goals.
POC have stronger community-based ties (“it takes a village”) so individualism (“I’m a man/woman at 18”) isn’t really the backdrop like how mainstream culture portrays it. Trial by fire doesn’t serve those three well without avenues to ask for help/guidance/mentorship.
More trust and goodwill is needed to convince them that their success translates to company success (rewards, recognition, letting them solve problems creatively, etc.). Just ask and listen….
Now, If you failed to assess their skills from the jump then the blame lies squarely with hiring managers who have misguided intentions - what else are they getting wrong?
Saying POC looks and sounds like Colored People. Racist as F**k. Stop defining people by their skin color. Period.
make sure you document all your attempts to improve their behavior or make them better employees and then fire them. You should not fire or go out of your way to keep bad employees just because they are POCs.
No need to worry about it.
Explore other options besides termination, like additional training, mentoring, or probationary periods if there's room for improvement.
