How do “commissions” on top of base salary typically work for pre-litigation associate compensation?

What should I do in a scenario where I was hired (12+ years) with far less salary than(~20% less) than other members (4+ experience) in team and asked to take 80% of team's responsibilities. The hiring manager low balled me sensing that I am in dire need of that job and now I am stuck for 2 years now. How long does it take for an insurance adjuster to review a demand letter and contact with an offer? Fault is not contested in this case. I sent the demand and records 3 weeks ago. They contacted me after I sent it to confirm they received it.

4th year at a mid size PI firm. What’s a good LA or NY business litigation firm and salary to expect? I’m a 2yr with quite frankly a lot of experience across civ rights, PI, excessive force, wrongful death, employment defense, K dispute, corp. structure dispute, professional licensure. Write every kind of brief, appeals, argue motions, take depos. Am the only associate working directly under partner on all cases. In house or M&A/Real Estate Transactional are likely the end goal but want to get the litigation out of me first.
