Cleared Accenture Skill interview by answering almost all the questions and received congratulatory mail for HR round and submitting all documents. Then after a week received a call from HR saying that due to some technical issue, the skills round will happen again and an online assessment was set up. In the assessment, questions which were not directly related to the profile were asked and then a rejection mail was received. Does this happen a lot?

Honestly, I woudl have stopped them in their tracks and asked them for clarification. I actually had this happen at a place I accepted a job at. When I got in the door it was actually a junior role. I ended up only staying two weeks and then leaving for another company. It was not a good ordeal.
The hiring manager knows the scope. If they call it a Jr. role, it is. It’s good context to have. You don’t want to take a role thinking you’re more Sr within the team than you are. Also, as you become a more Sr Mgr, there are a number of roles which you’d deem “Jr.” even though it might be Sr to your current position. Personally, depending on the topic, I’d consider a Vice President “junior” at my firm because they are not in a position to make certain decisions and / or have the right level of impact. I might need to defer to a MD…it’s all relative.
I would echo what SVP1 said - just because he mentioned the word "junior" doesn't mean it's technically a lower level. Every company is structured differently. It could easily be that the sr. level you're currently in is, indeed, equivalent to the jr. level at the new company. Best way to know for sure is to ask about it.
Ask for clarity. Could be as simple as she misspoke. Every company has different organizational structures as well.
I would ask them for clarity in the follow up email or if they send you an email for the next steps in the process.
I think context is important and personally, I wouldn't make any judgements until you see the offer come in. I think it's so relative based on the org structure that it's hard to say
I think you still need to check the job description and the role that you are applying for. You can also ask the hiring manager for some clarification so that it would be addressed right away.
