Why does a H1b visa stamping get rejected? Are there things I can do to avoid rejection?

I had received an RFE on my h1b LCA for new location for speciality occupation post filing a response it got denied. Did this happen with anyone else? My H1b extension is due next year n i m worried

Is that correct?

I have an approved I140 from previous employer and current employer hasn’t started GC yet. Been on H1B since 10 years now, so if my new GC doesn’t start before my H1B is up for approval in 2023 cont.

This one is for folks on h1b visa. So at time when you have an offer in hand(not accepted the offer yet and h1b transfer process not yet begun) while you are working with your current employer, do you even speak to your employer about your new offer to see if they make a counter offer?
Is there a risk involved with this approach assuming your current employer doesn't provide you with a counter offer and they ask you to leave immediately (and your h1b visa hasn't been transferred either)
