I mean this is hard...I think it's better to be balanced. Be open to growth when the opportunity comes, but it's okay to settle and be stable as well if you have something good going. I think you also have to face the fear of failure head on...if the risk is good enough, just do it.
Failures are not a bad thing. We are taught to always be concise and correct, but it's in our failures that we learn the most valuable lessons! I, too, am a perfectionist, and I have had to work my way through this. The reality is that your work is still valuable even you if you make mistakes. We are human, and everyone makes mistakes, and that's ok. Be confident in your work and your work ethic. New tasks and opportunities would not come your way unless you were capable of handling it. Growth often comes in times of feeling uncomfortable. If you want to continue growing, you have to be ok with that feeling! :)
Developing this mindset takes time, especially if you were raised differently. So be kind to yourself and patient & practice encouraging and affirming mantras. It can just be something small each day: "I did my best today & I'm proud of it." Talking to yourself this way will help you train your mind to appreciate the small achievements instead of looking for mistakes.
Challenges are part of our journey. Your learning process will never be complete without facing these challenges. And failing too is nothing but part of the entire process.

Just like what you said, you are being a perfectionist. I think you need to give space for possibilities of failure. That's the reason why you are having a hard time with the thought of failing. An achievement whether small or big is still called an achievement. Value and appreciate yourself for every effort you give.
Yes. You are right. I have been great at academics till Matriculation and there were some instances where I have had really bad experiences when I failed at something in school in early childhood. I have been compared all the time with others during my childhood by my parents . Not sure if that had made me risk averse and just trying to be stay out of facing fear of failure.
