Real short. Don't try and be funny in the email. Your name, what you do, and a link to your work. This is what I do and I get a great response rate from CDs
Don't go to the CD. Go to someone below them with more time.
Compliment their work
It’s a good question. I think if you really want a response it needs to be very personal and well written. Form copied and pasted emails are the worst.
I would say that I'm looking for genuine informational interviews, but of course hoping for more if the conversation warrants it. Sounds like I should start hitting up some ACDs, though. And of course I never take it personally, I know how busy things get for me and I'm just schlub Copywriter.

AC 1 & CD 2 - this is something I’d love some advice on too. Seems you both share very different opinions. Also, it may depend on the receiver. One recruiter I worked with once told me. All you need is 3 sentences if you have experience. Name and a little background (school etc). Agency experience and clients. Awards if any and website. Anyone thinks this is good?
Interesting to hear people say keeping it quick and to the point is key.
I believe I keep my outreach relatively concise, but it's definitely more personal note complementing their work than name, position, and experience.

@ECD1, I've been told that anyone lower than CD doesn't really have enough influence to push you along to the (hopeful) next step—a recommendation or an into to someone who can potentially hire you. Am I going in with too many expectations?
Is it a genuine informational interview or a hope to get hired? The two are different and if you turn an info interview into a job plea you will annoy whoever you're talking to. The thing to remember is that CDs are really really busy. If you go to someone below them and they think you're great they should advocate on your behalf. Happened to me many years ago. You should also get useful feedback on your book and can be more candid. Also don't be offended if CDs don't get back to you. They're not stuck up a holes who don't care for the little people. They're focused professionals with many competing demands.
