I need advice on how to approach a raise request.

I am a software sales representative in the manufacturing space. My starting salary was $85,000 in 2015 I did not receive a raise until I asked for one three years later in 2018. The raise was 5%. I have not received a raise since the 5% raise I received in 2018. They granted me a $2000 one time bonus in 2021 when I asked for a raise. How do I request a raise and get one?

I believe that I am being blocked from my transfer even though my manager approves it on paper. Is there any way I can find this out. I want out of healthcare so badly but as the sole wage earner in my family I cannot afford to start over in an entry level role in a new industry. I am so systemically exhausted I am struggling to translate my current skill-set to market myself into non- healthcare markets. When I say non- healthcare, I truly mean completely NOTHING to do with any direct clinical or patient related/facing anything. Anyone have any worda of wisdom or insight?
