I have worked in industry primarily and I can tell you this can be a red flag, especially if the blame is coming from your superiors and they have accounting backgrounds. In this case, it's a form of denial either to the extent of the problem (which can be a people, process, or system breakdown) - If this persists after you have clearly articulated the problem, it maybe time to quietly look and see what's out there as not all organizations in industry are like this, some actually have reasonable people who are not blame mongers.
I had a similar situation, whereby our inventory people where not following procedure - so some high value inventory was not in system and not on our books - which caused major issues both on b/s and p&l - and it was a recurring issue even after adding additional controls and meeting with warehouse folks and every time the blame fell on us the accountants. It was a clear case of a management that was in denial of the real issues.
All in all, you have to assess how bad it is. If it's one occurrence then you're fine, but if this is recurring that's red flag number 1 and if you superiors have accounting/big 4 experience that flag number 2 - as they should understand these concepts - before but certainly after you have clearly explained and articulated where the challenges are emanating from. The accounting practice was not designed to capture all mistakes and errors - especially those done by others - is this not why auditors (I'm not an auditor) - state in their opinions that they can only give assurance to the extent that the f/s are free from material misstatements but they can not give assurance that there is no fraud and or other issues - as no one can vet everything.
Write up a business case showing the monstrous cost this brings to your company and what you can do about it. Show a 5x return on investment vs cost savings and you can create an audit culture in your business.
Yeah I know that sounded funny but seriously anyone with passion and political capital in and organisation can make a case for just about anything.
I feel the same way.
