If there is a sought after Director for a practice being built up at PwC. Has there ever been a time that a director can go straight to partner if they have an external offer that is quite compelling ?

I am looking to make a career change to HR. I currently make 75K as a sales and development coach. Can somebody explain to me how the system works between payer (insurance) and therapists? I’m currently looking for a therapist with a specific focus due to a traumatic event and am only finding counselors that do not work with insurance and charge average $ 150/hour. I realize fair payment is important but don’t understand how people can afford to do it out of pocket as “regular mortals”...
