Hi! I had hormonal acne forever and finally went on accutane. I would try Spiroloctane first if you haven’t - it’s amazing for hormonal acne and most people do really well on it. For me, those side effects were worse than accutane but I’m sure I’m an exception. As for accutane, I’m done in a month so can’t speak for long term but so far my skin is perfect even during my period and I’ve had no insane major side effects besides mild join point and muscle aches which my derm said is common if you’re very active. There’s also a newer brand called Absorbica that is gentler on the stomach apparently

Def see a Derm if you can! They can put you on a topical like Tactuo (Epiduo in the states) which is a combination retinoid + benzoyl peroxide, or Biacna (combination retinoid + clindamycin), or Aczone (dapsone gel). You could also try differin (available OTC in the states). BHA type products will help with clogged pores, but if you have those deep cystic pimples, you would be better off using smthng with a retinoid.
Also if you’re on Roaccutane, i believe you’d have to go back on the pill...
Definitely try everything else. A combo of tretinoin and aczone worked well. My issue was my zits just came back after a long time on each topical and spiro made me too light headed. I’m really glad I did accutane but it’s definitely not the only option. I just had such persistent deep zits and I was over it lol
