Correct. I am not the advisor on the plan. So the proposition is I'm overseeing your 401(K) investments, fund selection, asset allocation etc. Quarterly rebalance and most importantly keeping you making good decisions with the account.
I think it needs to be flat fee regardless of account size. Does it really matter if it $25k or $25million if all your doing is asset allocation and a rebalance? The behavioral aspect I suppose is where you can try to crank that few up.

Yes. With my b/d it can be done with a flat fee advisory agreement. I've charged up to $300 for this in the past. I've thought of marketing the service more at some of the bigger employers in the area as a way to get known and be the 1st guy they call on a Rollover in the future. I'm not sure how to go about it...
Is this what you guys mean when you say cold calling? HR directors might be a good target for this but how do you get them to let you in their door?
