Sounds like something you should disclose, just a matter of to whom. If workplace knows, they are obligated to provide reasonable accommodations for disabilities, this might be more frequent shorter breaks, less travel, etc. The key is "reasonable" and you'll likely feel the vibe of whether it's going to work long-term. Might want to check with your Dr to see what has been useful/granted to other patients with similar demands. Otherwise, might as well jump ship, company, profession, both?
