I think you’re at the point of cutting ties. I’d go to HR now
OP we need a timeline on at least what’s happened up to this point as far as the time around the steps that you’ve taken. If you’ve exhausted everything that you can that’s the first question that HR should ask. If that has been done, then your next step could be to look for a better position for the employee because clearly they’re not cut out for this one. If there is no other position, then corrective actions need to begin.
Tough spot. If coaching isn’t leading to progress, how long can the team afford to carry them? Have you set a clear deadline for improvement?
How long has it been?
Why replace them? Why not just reassign them to another project? Or give them tasks you know they’d be good at?
You say in your post you want to be patient. Then be patient. It’s not easy to be patient and life will throw people and situations that will test your patience- that is precisely when you have to be more patient

I'm a very intuitive person, I pretty much know after a few interactions with the person whether it's fixable or not. The "coaching" is just to cross my Ts and dot my Is to prove out my intuition in reality.
Every great once and a while someone surprises me, but you can usually tell based on understanding someone's real personality and motivations what's going on and if it's salvageable or not.
