Hi everyone! I currently work in an outpatient setting doing individual therapy but am planning to leave my job. I have a handful of patients, some of which have been meeting with me consistently for over 2 years, that I suspect will want to remain in touch after I leave. Has anyone ever done this before? I’m maxing out the 6th year for H1b and my I-140 just got approved. 1 month ago, my employer has filed a h1b extension but it is based on the approved PERM instead of the 140, and only asked for one-year extension. In that case, after 6 months (180 days i140 rule to avoid employer’s revoke), will I be able to switch employers even though the renewed H1b gonna expire soon at that time?

Immigration lawyers had sent me a questionnaire to fill out for Manager PERM. They now just came back saying that they need to assess me for a different position. Anyone else experience this? I wonder if this means they’ll now just file a Senior Perm instead? Asked them but they’re not very responsive.
