What level are you at bcg? It’s not too soon if this is the role you’ve been waiting for. Why wait longer just to risk not getting the job later (if this is what you actually do want)?

Whoa startup paying that much? What funding stage is this company in? I’m curious because I’m also interviewing with a startup.
Consulting is not all that anymore and arguably leaning curve is even more steep as you get wear so many different hats. I have been in consulting for ~15 years and wish I I left the industry sooner.
Take it. Heathcare AI is probably the hotest space right now. Great offer considering they are in a mid-stage growth and you can make a meaningful impact. The equity option alone will make it worthwhile if the shop goes public a few years or get acquired. I am looking into the tech space. The comp base tends to be low in most start-up's because they want to conserve cash. The incentive is the valube of their options, which is a gamble. If you see the potential in the maket and understand the value of their products and it attracts good talent, make the move.

As others have mentioned, the door is usually always open to return to consulting firms after you leave (on good terms).
Another way you can think about is through the lens of mininizing regret. Everyone's calculations look different, but I think I'd regret missing out on {cool, new, exciting opportunity} versus missing out on {more of the same job I've already been doing}
