Don’t quit. Save money, don’t sweat over it and let them fire you. That way you can collect unemployment.
You need to see a therapist and request for FMLA. Take a month or two off with medical leave and reevaluate your situation then/after.
I am also VP level and just did exactly this. It has been a transformative experience. In the immediate, I’d recommend exploring FMLA but what it really sounds like you need is to GET OUT - no job is worth your sanity
Unfortunately, when you get a sign-up bonus you normally have to agree you will pay it back if you quit before 12 months. Most companies will enforce the agreement- unless they let you go. If you quit, you don’t get severance anyway, so that’s a moot point.
I was so miserable at one company that had given me at $30K sign-up bonus; I couldn’t leave, but I dreaded every day. One month before my year was over, the company had a massive layoff. I was practically euphoric - not only did I not have to refund the money, I got a month’s severance.
Now, if you actually have a medical situation - like being diagnosed with a nervous breakdown by a doctor - you probably won’t have to pay it back. Good luck!

You may want to look into a medical leave of absence.
I'm sorry you're in this situation. Can't be sure about the contract, but just assume the worst and that you'll have to pay it back. You'll be okay. Regardless, you should quit. Just a matter of when/how.
Before you pull trigger, make sure you have 4-6 months of cash runway/emergency fund. You won't be able to properly decompress if you're worried about getting a new job immediately.
If you don't have the appropriate runway, I would highly suggest finding ways to mitigate the stress via other means while you figure out your exit, but I wouldn't quit yet. If you spend less than 15k keeping yourself sane, you'll still net a smaller loss than if you were to quit on the spot.

I know someone else at your level who went through the same issue. He sought therapy. He learned to manage what he can and can’t control. It did wonders for him and is still employed at his company.
Most places make you stay at least a year to NOT have to pay back signing or moving bonus.
I am so sorry it’s been so rough! I have been there and your family and your health come first.
Is there anything you can do to reclaim some balance? Your company will never help you with that… but can you do less? Or, match the output to the time you have?
