That's bizarre. I would probably decline citing the lack of interaction. I wouldn't feel comfortable providing feedback in that scenario.
It's best to sidestep that however you can. You don't want to be in the position of evaluating someone if you honestly don't have sufficient information to pass judgment.
I would probably do the best I could, and that would include the fact that my experience with them has been minimal. I remember getting feedback from someone I barely knew or worked with. Funny how that happens, and how little good it does in the big picture.

I informed my manager when that occured they asked I still complete it based on very limited interaction.
The brief interaction I had was pleasant but I could not speak to work ethic or their contribution to our facility goals. I just made to sure to include that our time working together was too brief to appropriately evaluate.

I ended up doing the best I could, but it was barely a review.
Agree with others, say that you don’t have much experience with them but say whatever you can.
Are there other direct supervisors you can speak to regarding work performance? Otherwise I would do the best I can based on what I can see or revmore- documentation , attendance, etc.
