I still think OP should look at other options before reporting the woman after two days. Especially if they’ve got a decent amount of lead up time until it’s due. If played right, OP might have an opportunity to stand out as a team player and a strong leader. Regroup, make a contingency plan, get it done.

Reach out to her manager (which I hope is not you). They can decide if this needs to escalate to HR.

If you see something as a pattern then it’s a reason to be concerned about underperforming but if it’s just been for a few days she could be unwell. You could offer to help as you ask for turnaround. But if you really feel like the pregnancy is used to underperform maybe then you can bring up the performance issue and leave out her situation.
I think first you have to realize she is pregnant, and how far along she is. Is she is 8 or 9 months, she is tapped out. I would ask her if she would like to do another part of that project or have someone assist her with it. She may feel as though she can't do it, but doesn't know how to say it's too much due to her condition- pregnancy, and doesnt want to ise that as an excuse. Keep in mind pregnancy is not handicap an is a protected under FMLA so you may want to talk it through hypothetically with your manager or your mentor. But try setting up a meeting with her to see what is the issue or how can you help her. It may be best to have someone else work on the project with her.
What does her being pregnant have to do with her underperforming? Take it from someone with a background in HR, focus on her performance and establishing a pattern of a lack thereof if it exists. Do not mention anything related to her pregnancy.
Babies come early, pregnancies can become complicated/high risk, people go to checkups and learn they’re having twins, give her some grace - you’ll find yourself with a serious family situation arising at some point in your career and others are going to pick up the slack.

As a woman who has had multiple friends deal with pregnancy, miscarriages, placenta abruptia, and pregnant while experiencing domestic violence, I know that sometimes it helps to have an understanding team and a manager who is good with catching other team members up to back fill and support. Some women need their jobs, their lives and their children’s lives depends on it for security. But all you see is a cog you failed to have back up for. Why didn’t you have a team member shadow her?
Your managerial style here is the fault.
