If you like the new role and you will produce good work there, sometimes you have to jump around in advertising to work your way up the ladder and get big pay bumps. You can always go back to another big agency. You might gain valuable experience in the more senior role. R/GA will look good on your resume. Leave RGA on really good terms and you could go back in a few years.
You can work at an amazing place and make shitty work. You can also work at a lesser known/unknown place and make amazing work. All that matters is how good your work is, not which building you sat in to make it.
The good news is you already have R/GA on your resume. Now use it to get better opportunities.
Ex-R/GA here. Ok, as long as you don't fuck up your departure (do something to make them mad like going to MRM or AKQA,) R/GA will take you back as a boomerang down the road. They might even promote you on your way back in. I've seen it a zillion times.

Depends where you're at in life.
I wouldn't personally go into healthcare as I feel like it's a totally different industry but you already have RGA on your resume. Going somewhere smaller and less known is not career suicide.
Depend what the role and companies involved are.

Brand name companies on your resume are most helpful when looking for a new job: the new companies will pay more because of your name brand work experience. On the other hand, name brand companies know they are the 💩, and that you'll stay for the name, hence they pay less.

Yes. Money and experience lead to more opportunities. You can always go back to brand name agency.
