Both. At some places a PIP is actually trying to help. In others, it’s just paving the way to your exit.
This: some places do want to see you succeed and stay along after a PIP, but it’s legal CYA for when/if they fire you, regardless of if they want to actually see you improve.

PIP always means you should be looking for a new job, even while trying to improve your performance. Often, the PIP "goals" set by management aren't realistic in the specified period of time.
Pip now days means you are next when there are hard decisions to be made. Start looking
