@ACD1 As the team leader, you should send a polite letter/email detailing what the team finds objectionable (they all worked hard on the campaign, their work, you were around for 3 weeks vs 2 years, etc.) and request politely that they modify their book to reflect the actual work they did on the campaign (layouts whatever.) If they don't, explain that you'll have to request that the recruiters not place them at your agency anymore. If this person is wise, they'll understand that this is a small industry and word about this kind of thing gets around. I think most creatives know that recruiters talk to other recruiters and bad behavior comes up.

I don’t work with him anymore. And he doesn’t specify his role in it, that’s what pisses me off. And I bet he is gonna get hired as a CD somewhere just because of this. And no one will know. Credit whores are the worst kind.
Don't sweat it! Even if he lies his way into a job, keeping said job is a different story. If his skills aren't on par with what his book shows, his days are numbered anywhere he gets hired. I've seen it happen a few times. Besides, the ad world is smaller than small. Eventually such people develop a bad rep, and then it's their entire careers that have their days numbered.
^^^ yes. Exactly right. These people get busted eventually. Agree with LB1 too; try reaching out and see if he will at least add the proper credits. Also you're not putting his name on the award submissions, right?
Talk to your CD about your frustrations and let them handle it.
Just let it go, if you don't work together anymore there's nothing you can really do. If it burns you that much, reach out to him and ask him to take it down, but you can't really force him to. He sounds like a shitheel, any companies that hire him will find that out soon enough.
One day someone will ask you what you think of so-and-so and that's when you can tell them the real deal. Advertising is a small world and names get around.
Get over it. Make even better shit and you won't remember cuz you'll be promoted and make more money. It will always happen and don't forget lots live under a shadow
