Many things. But most important one is that even with advance medical procedures, tools, and new life saving treatments, it's hard to educate all medical professionals to adopt to something new. Even when you get them to adopt it, the service/product provider wants to charge them their patients arm, leg and a kidney. Of course the insurance steps in an has a say whether they want to pay for it or not and they think you really need it. At the end of the day, it just comes down to money.
