For me, it's people. If I hate the people I work with, there's nothing that can motivate me to go to any agency thing... no matter how cool it could be. Also, when someone tries to "create culture" it comes off as really disingenuous... especially if they aren't good at it.
It happens but you need people who can see farther than their nose which happens once in a blue moon in advertising
Culture just happens (or doesn't!) based on the mix of people you have. If you try to manufacture it, you already fucked up.

It can be created but it needs buy-in and everything the company does needs to ladder up to the mission and values so that everyone is living by them daily. It's really hard in client service organizations because the work doesn't provide your own company value, it provides value to someone else with other/different values or mission.
It's about people and personalities and of course skill sets. If you're leading a team at an agency and have to deal with HR it will take you 3 years minimum to build the group you want. You can operate much faster if you're independent and have hiring authority.
I miss people who made my work seem like the most fun I'd have in my life. Those are precious
The best agency culture comes from fun, hardworking people that aren't afraid to hang out after work. If you never want to grab a beer (or coffee for the non-drinkers) with your coworkers, it's a good sign the culture sucks.
Location is also important. LA is horrible for culture because of the commutes. But definitely not impossible (particularly at smaller shops).
