Don't apply to work for a judge if you wouldn't take the job.
Yes, you’ll be fine. If it’s awful (it won’t be) you’ll be a better lawyer and have better career opportunities.
Not lore, but you should have a truly compelling reason to decline (family, finances, etc.). Otherwise, you should be prepared to accept.

Yes, you should be prepared to take it. Do not apply to a clerkship if you are not ready to take it.
I was offered my clerkship during the interview. I feel this is common if you are the best candidate and you hit it off with them.
The prevailing custom is to not turn down an offer. I wouldn't apply to a judge unless you want to work for him or her and think the timing is right. But, a job is a serious commitment and I do not think candidates should be pressured into something they think is a bad fit because of a power imbalance. Personally, I would be comfortable turning down an offer if something gave me the ick. Some judges might not take that well, but those also probably aren't the judges you want to work for.
