I’ve had this come up with one of my team members. I’ve provided them with discrete steps to guide their thinking on supporting client efforts. Essentially to help them think deeper about problems and ways to resolve them (like a funnel). They should go through this checklist each time they work on something. Also, back to the basics - a running to do list for even the smallest of tasks, adding calendar invite reminders for themselves is also an option.
This doesn’t address each item from OP. But the thinking I find to be the most challenging to overcome. With organization that comes with preference to a degree. Being proactive comes from understanding what is needed or should be needed, which goes back to thinking.

I ask them to think about how they'd want someone to present information to them. I think sometimes we can be a bit obtuse but when you step back and say "how would I want to consume this info" it can help.
I'd also give them a QC checklist and tell them that literally their JOB is to make sure you as their boss or whatever aren't caught off guard, so make sure to share info that will make sure that doesn't happen.
I find that putting things in terms of "this is WHY" can be helpful context.
