So i started with a new cliant. My boss informed me that she was bed bound and could move herself with some assistance when I got there I found that not to be the case. I thought the whole point of having an HHA was so you could be independent at home. She has no hoyer lift and it takes 2 people to move her. The family just dumped her at home and left. She has a neck brace and is completely immobile. My boss said she may need skilled nursing but they are refusing what do I do?

I've been in home health care my entire career. Call your agency, report your concerns. Tell them you feel unsafe and you no longer wish to be assigned to this case. Be sure to stand your ground. Your boss will be responsible for reporting concerns regarding the welfare of this individual. Get a new assessment, leave the issues up to your higher-ups.

Sounds like a disaster waiting to happen. And, a potential liability to boot. Personally, I’d tell my boss I was uncomfortable with this assignment and will not to be put in an impossible position. Your boss needs to talk to the family, imo. Good luck! 🍀

Is this person elderly? Do you have a social worker at your agency? Honestly, it sounds like a case of neglect if the decision-making is being completed by the family. However, if this is the patient's wishes then everyone has the right to make bad choices. Maybe the family is fed up if your patient doesn't want the help. Either way, it should be investigated further and I assume as a healthcare provider you are a mandated reporter.

A situation where everyone seems to need to get together and be on the same page. It is not ok when it comes to the needs of the patient to be inaccurate and putting you in a position where the reality is completely different. Your boss or whoever needs to communicate with them and get it figured out.

I've hired and supervised CNAs in home and adult day programs. I tell my staff to never take a risk with their own health and well-being. Do not try to transfer this client without the appropriate resources. If you get hurt, then it WILL impact your ability to work and potentially could create a long-term injury. There needs to be a Hoyer! It also sounds like a protective service situation. The supervisor and social worker (if there is one) need to get out there, assess the situation, and make a protective service referral if it looks like a safety issue. Do not put yourself at risk!

Ask for a different client to do. Unless their going to let another employee assist you with her care. Especially since it takes 2 people to assist her without a lift. That's hard work doing a person who's dead weight. If you try to do her by yourself your body is going to be in pain for days.
