That's a judgment call. If someone said something and apologized, and you accepted the apology, I would say that ends it. There's no need to put it on the record. But, if you feel the apology wasn't sincere, or if the same thing happens again, that would change things. And in that case you might want to escalate it.

I would flag it. I don’t tolerate any BS at work. We “have” to be there 40 hours a week and the last thing I can stand are the people who try to make that experience unpleasant for others. I’d make it a point to say something to both my manager, HR and document it personally.
