Country Club membership used to be common with old school executive benefit packages. But for the average employee? Pretty rare.
If by “Health Club” you are referring to gym memberships, it is a bit more common to see it administered as a discount program to save on membership fees.
I've seen gym memberships offered as company perks, not so much country or health clubs though. Wellness stipends seem to be growing in popularity too.

Gym or health club memberships have been a benefit for decades, though they're probably becoming less common as companies focus on cutting costs. Country club memberships paid by companies used to be a status symbol, and I suppose they're still offered in some pay packages. Every now and then an executive's pay package is written about in the press and it can be startling to see what's included.
No, I’ve heard of employers giving discounts to fitness clubs and to be honest I’m not sure the difference between a health club and a fitness club is but I’ve never heard of a company giving benefits to sign up for a country club. Maybe if you’re really high up there in the company ranking.
