I agree that’s it’s overrated. But that doesn’t mean it’s not good. There are better places in SD, but OP asked for downtown/little Italy recs

Morning glory and Nolita Hall are my favorite ones.. but there are so many others!

Little Goat or Cira. One of the best places to visit for breakfast. They have the best chefs giving you the best tasting food.

It's in San Diego. More specifically in Little Italy.

Hi there! Caffee Italia would be a good place. It's my favorite breakfast place.

I recommend Farmer's Table! Food is amazing. The service is also excellent. Hope this helps.

Queenstown Public house! New Zealand themed; this renovated old home to restaurant feel is spectacular and the food is KILLER. Highly recommend checking it out; but don't go with too large of a party.

Cavatappi in Roasted Red Pepper Cream Sauce

Tired of of the same old boring tomato marinara sauce? Try a roasted red pepper cream sauce!

Red bell peppers are roasted in the oven until sweet. It is then blended with caramelized onions, garlic, red pepper flakes, sun dried tomato paste, and cream into a silky, smooth and velvety sauce. Finish the sauce with a knob of butter and coat your pasta. Cavatappi works well here. Don’t forget to shave a little Pecorino Romano
