Don’t tell them in advance. Take your leave and when you’re getting close to the end of it, if you still feel this way, then let them know.
Be prepared for possibly having to pay back medical coverage from during your leave. Check your handbook!
Don’t tell them in advance or they will let you go. If you have health insurance via your company and your leave includes unpaid time - you may have to pay that back since you didn’t have a paycheck that they could draw from for a period of time.
I did this same thing and gave my notice to my immediate boss, the same as I would do if I was quitting to go work somewhere else. Then I wrote a standard resignation letter and emailed it to her and she fwd-ed to HR. The initial conversation was hard but the rest was super easy.
I took a year off and went back to work and it was the best decision I ever made.
