How much time do I give a clinical leader to learn their position? She comes with experience but not leadership experience. A big part of the position is to put out fires but she seems to lack the critical thinking capabilities to do so. This is a start-up and requires a high level of commitment, which she was aware of, but she is not willing to go above and beyond. It's been 6 months, is it time to demote?

What resources does she have? Was she given proper training? Before putting all the blame on her, was she provided tools to succeed and shown how to be successful in the role? If yes, then possibly time to demote. If no, then I would give her the chance to restructure/relearn the role.

Read "The First 90 Days" with her and make a plan based upon its guidance. Yes, it refers to the first 3 months, but start planning while you read together (there is specific guidance for startups). Developing a plan together using this proven method will give you both a much more clear roadmap. It would have been nice to have this in place from the beginning, yes; however, you can start now and do it right.

Has she been provided with the necessary training and protocols to know what to do in different situations? If so and she isn’t doing well, I would be having some 1:1s dedicated to expectations of her role and what areas she needs to improve on to be successful.

I highly recommend working on setting up something similar to a 30-60-90 plan with her. Sit down and come up with this plan together so you both can be on the same page on what she expects she can accomplish within the next month, 8 month, 9-month mark, etc..
